Community Care Coordinator

The Village Baxter Community Care Team provides funded and private In Home services throughout the Frankston and Mornington Peninsula area and is looking for an additional Coordinator for our growing team.

The Community Care Coordinator is responsible for the day-to-day coordination of the Home Care Services Department, which works to assist clients in living independently in their homes and community, in a dignified and safe manner. This role plays a critical part in facilitating and coordinating provision of Home Care services which are of optimal quality, highly flexible and responsive to client’s needs.

Experience with rostering, ideally within an Aged Care or Disability services setting, is essential.

The successful applicant will have:

outstanding communication, interpersonal, organisational and time management skills

a commitment to excellence and high-quality care

an ability to work independently and within a team structure

current unrestricted Victorian Drivers Licence and roadworthy insured vehicle preferred

Pay is weekly, under the SCHADS Award, with a 3.5% pay increase applicable on 1 July 2025.
