About the Role
To provide advanced specialist clinical and professional practice, facilitating a safe and efficient patient journey across the continuum of care through applying the Nurse Navigator principles of coordinating patient centred care, creating partnerships, improving patient outcomes, and facilitating systems improvement. The role improves complex patient care, increases system efficiencies, reduces costs by streamlining and coordinating care. The General Practitioner Shared Care Nurse Navigator (GPSC) will allow the continuing involvement of the mental health service and General Practice in the care of the patient and aims to coordinate patient care to provide high quality integrated care that is readily accessible and convenient to the patient and cost effective for all parties involved.
The role requires that the Nurse Navigator participates in reflective clinical practice, mentorship/clinical supervision monthly.
- The role requires highly specialised clinical, nursing skills, knowledge and experience in order to effectively provide, manage and lead contemporary generalist and mental health nursing practice and health services.
- The role works to support consumers with mental health concerns and complex physical health co-morbidities to access appropriate mental health and general health care in the most appropriate setting. Referrals are accepted from primary care providers and via internal referral pathways for open consumer of the community mental health services.
