An exciting opportunity for a Senior Care Finder to work across a program delivering specialist and intensive assistance to people within the Western Sydney Region. This role will support and encourage older Australians to make informed decisions about their aged care services, support and facilitate assertive outreach to build individual and community capacity in communities. Ideally Candidates will be centrally located in The Hills or Blacktown Shire and surrounding suburbs.
We offer a full-time permanent contract, working Monday to Friday-no weekend or shift work required.
To ensure you feel confident and supported in your role, we provide a comprehensive two-week training and orientation program. The first week is in-person at our Coolangatta office with travel, accommodation and meals fully covered as per our travel policy. Our next orientation intake begins 14th July 2025.

The role:
- Working collaboratively with clients with complex needs by assisting them to navigate through their aged care journey
- Facilitate assertive outreach, reaching out into local communities and actively engaging with potential clients through direct contact and/or working with appropriate organisations
- Participate in building community capacity through attending network opportunities and working collaboratively with others
- Ability to be self-motivated and work autonomously to meet outcomes
About you:
- Experience in assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services
- Knowledge/experience in person centred, trauma - informed care
- Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach
- Local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region
- Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite)
What to bring?
- A minimum of 5 years' experience working within the aged and/or community care industry
- Tertiary qualified Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker with AASW Registration
- Unrestricted Australian driver's licence
