Hi there! We're Five Good Friends, and we're an approved aged care and registered NDIS service provider. We help our clients (we call them Members) live vibrantly in their own homes by wrapping a care team around them and their families. Our software coordinates in the background so our teams can do their best work. Our carers (we call the Helpers) play a key role in creating the magic for our members. Five Good Friends strives to form high-quality, long-lasting connections between Members and their Helpers.
About the Role
As a Helper (Carer) Planner, you will work in our Welcome Team, reporting to the Team Leader. Your ultimate responsibility will be to achieve the perfect match of support workers (we call them helpers) during onboarding for our new members, so they receive consistent quality of care and support each and every day.
Manage and respond to Helper Request Tickets, ensuring timely updates and action tracking.
Match Helpers to Members based on preferences, verification requirements, and compatibility, using internal systems and the FGF App.
Communicate with Helpers and Members throughout the matching process, encouraging engagement and providing regular updates.
Plan and maintain effective rosters, coordinating multiple Helpers where needed and ensuring continuity of service.
Collaborate with internal teams—including Care Planners, Onboarding, Rostering, and Recruitment—to support seamless service delivery.
Monitor and maintain accurate Helper and Member data to ensure effective reporting and support informed care planning.
Provide back-up rostering support and assist with filling emergency or temporary visit needs when required.
Do you have what it takes?
Successful completion of undergraduate degree or relevant qualification in HR, Community Services or Health Sciences would be highly regarded but is not essential
Customer service experience
Care co-ordination in a home care package program and/or NDIS funded support services is a plus
Ability to communicate in a professional and friendly manner
You keep sight of the bigger picture, always working toward it and helping others see it too.

You are reflective and if things go wrong, or miss the mark, you lean into it to learn and improve for next time.
You love solving problems and finding innovative solutions.

Flexible work options – this role is based in the head office in South Brisbane, however flexibility to work from home a few days per week can be provided.
