About the Role
As a Triage Delegate with the Central Adelaide Aged Care Assessment Team (ACAT), you will play a vital role in supporting aged clients through the intake and triage process. You will manage incoming referrals, using the Integrated Assessment Tool (IAT) and My Aged Care client records to determine eligibility and clinical priority for a comprehensive assessment. This includes accurately entering data into the My Aged Care system and providing clients with clear information on available aged care pathways. Whether it’s community care, residential respite, or flexible care options, you will guide clients through their choices and ensure they receive the services most appropriate for their needs.
In this dynamic role, you’ll engage with clients, their families, carers, and a range of health professionals, ensuring a seamless and compassionate service. You will be responsible for adhering to the Aged Care Assessment Programme guidelines, meeting key performance indicators (KPIs), and participating in educational and quality assurance activities. By aligning with legislative and funding requirements under the Aged Care Act 1997, you will contribute to the continual improvement of services in line with SA Health's Aged Care Strategy, making a real difference in the lives of South Australians.
