Client centred home care

CCHC is not like other places you've worked. We are a rapidly growing organisation and we are doing extraordinary things for those living with a disability. We are committed to offering a friendly place to work and a genuine and supportive team environment. We want to provide a ‘life changing’ experience not only for the people we support but for all the people that work with us.
Together we are driven to be thoughtful, practical and purposeful in all that we do. If you want a career where you can thrive, be challenged and do meaningful work, you are in the right place!
We have a vacancy for a Community RN Nurse to undertake a regular nursing run. The nursing run is where you visit clients in their own homes to undertake wound care and catheter management and you see multiple clients on the one day. We have a couple of days a week where we have nursing run shifts available. For the nursing run, suburbs that are included are Deception Bay, Strathpine and once every three weeks you attend to a client at Gaythorne. The nursing runs are finished by lunch time or early afternoon so you have the rest of the day to yourself! Whilst you are casual, we offer you regular shifts. The shifts that we have available are for a Tuesday and Wednesday. If you have flexibility to pick up additional shifts, that would be highly desirable!
Our clients have complex health and high intensity requirements. We offer our nurses (even though you are casual) regular shifts, with regular clients so you know who you are going to see and what it is that you are going to do! We are seeking someone to do a nursing run – where you see multiple clients in the time allocated, in their own homes and you get paid for your time and kilometres between clients. Depending on your skill level, from time to time, we do get referrals for nursing assessments and reports in that area that you could pick up on an ad hoc basis.
We provide full training and support by a Registered Nurse and also offer training through our online training platform.
Key Responsibilities:
Follow the care plans within your scope
Undertake tasks such as PEG, IDC, SPC, pressure area care, wound care, diabetic management and medication administration.
Escalate any changes to the RN Clinical Case Manager or Clinical Care Team Leader.

Our perfect candidate will ideally meet the following criteria:
Excellent communication and clinical problem solving skills
Confidence in managing your time effectively, including the ability to work autonomously
General nursing experience, post-registration
NDIS Working Screening Clearance or willingness to obtain
Full, unrestricted AHPRA registration
A reliable vehicle, current Australian drivers licence, car registration and comprehensive insurance
We pay above award rates, starting at $50.00 - $54.00 per hour, ordinary week day rate
Saturday $70.00 per hour and Sunday $86.00 per hour
We encourage professional development and education and support for our people to grow and flourish!
If this sounds like you, please apply! Applications will be assessed as they are received.
