An exciting opportunity exists for a Physiotherapist-Senior, Geriatric Evaluation Management and Rehab Services (GEM/R), Hospital in the Home (HITH), to deliver high level Physiotherapy clinical services and leadership across one or more specific work units, to stakeholders/customers of the Sunshine Coast Hospital and Health Service and Queensland Health (SCHHS).

The SCHHS Hospital In The Home (HITH) initiative is a 36-bed multidisciplinary (MDT) hospital avoidance strategy that provides acute care to admitted patients of the SCHHS in their permanent or temporary residence.

The SCHHS Geriatric Evaluation and Management/Rehabilitation Hospital In The Home (GEM/R HITH) initiative is a 8-bed multidisciplinary (MDT) hospital avoidance strategy that provides sub-acute care to admitted patients of the SCHHS in their permanent or temporary residence.

Both the SCHHS HITH service and the GEM/R HITH service work to alleviate bed pressures, increase patient flow and improve patient outcomes by delivering care to patients in their home.

Key Responsibilities include:

• Deliver independent high level Physiotherapy clinical practice with infrequent need for direct clinical practice supervision. Exercise professional judgement to a complex caseload, adapting routine clinical practice based on valid and reliable evidence.
• Provide high level clinical advice, education and influence towards a common patient-centred vision and direction to professional operational teams and relevant stakeholders regarding service delivery.
• Provide leadership oversight and clinical practice supervision to less experienced Physiotherapy practitioners and students as delegated.

Remuneration value up to $157,359 p.a., comprising salary between $128,169 - $137,918 p.a. (f/t), comprising salary rates: $64.64 - $69.55 p.h. (p/t) employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (Fixed term temporary full time and/or part time positions available for up to 2 years).
