About the role

SAVITA Healthcare Pty Ltd is seeking a talented Support Coordinator to join our team in Melbourne, VIC. This casual position provides vital coordination and support services to older adults and individuals living with disabilities, helping them access the care and resources necessary to live independently and with dignity.

What you'll be doing

Assisting clients to access and coordinate a range of support services, including allied health, personal care, and social/community engagement activities

Developing and regularly reviewing individualised support plans to ensure clients' evolving needs are met

Liaising with service providers, government agencies and other stakeholders to advocate for clients and facilitate effective service delivery

Monitoring clients' progress and wellbeing, and identifying opportunities to enhance their independence and quality of life

Maintaining detailed case notes and contributing to the continuous improvement of support coordination processes

What we're looking for

Relevant tertiary qualification in a community services, health or social work discipline

Minimum 2 years' experience in a support coordination, case management or similar client-facing role

Excellent interpersonal, communication and problem-solving skills

Demonstrated ability to work collaboratively with a diverse range of stakeholders

Strong organisational skills and attention to detail

Empathy, patience and a genuine commitment to improving outcomes for older adults and people with disabilities
