The Farm in Galong (The Farm) supports women suffering systemic disadvantage with a focus on women whose children are in out of home care. The program provides several interventions to address a range of substance abuse issues and particularly the underlying causes of addiction. The goals of the organization are to give the clients the best possible chance of a happy future with their family. In real terms, this means the women leave The Farm with:
- A nationally recognized qualification that will allow them to find a job.
- Custody of their children, confidence in their motherhood and a plan for the child’s future education and needs where possible.
- An improved relationship with their wider family (e.g. siblings, parents) thanks to facilitated reunion sessions.
- Access to a new home, a new support network and employment so that we know they are going from a position of strength to strength.
- Usable life and wellness skills, including knowledge of exercise, nutrition, budgeting and communication.
Qualifications & experience
- Relevant health and or welfare Tertiary Qualification such as Social Work, Psychology, Counseling, Nursing, Allied Health Sciences, Alcohol and Other Drugs or other relevant area of study desirable • Certificate IV in Alcohol and Other Drugs or completion of the core competencies (or able to complete within the first 12 months of employment).
- Experience in the provision of assessment and counseling services preferably with Alcohol and Other Drugs or dual diagnosis clients.
- Ability to collaborate effectively with clients, families, colleagues, stakeholders and other service providers.
- Proven skills in the application of assessment, case coordination, case management, case formulation and the development of individual/family inclusive treatment plans.
- Proven ability to work with a range of people and families experiencing problematic Alcohol and Other Drugs use, in a non-judgmental and respectful manner.
Tasks & responsibilities
The case manager is responsible for conducting comprehensive assessments, person-centered planning, care and advocacy support and providing a range of counseling interventions to assist clients to change harmful behaviors resulting from problematic alcohol and other drug use.
The key objectives of this role are:
- Conduct comprehensive assessments, develop treatment plans and provide effective counseling interventions as well as advocacy to meet the individual needs of clients, including those with complex needs.
- Provide individualized alcohol and other drug assessment, comprehensive case management, individual interventions, realistic support plans and discharge planning
- Manage intake, screening, and referrals
