Case Manager - Homes First Program
The Homes First Case Manager will have a clear understanding of the Housing First principles and assist individuals or families to obtain and maintain safe and stable long-term housing. This will be achieved through thorough needs assessments and effective support planning for each individual family member (including accompanying children), providing practical support where necessary, and connecting individuals and family members with the Homes First multidisciplinary team or with appropriate additional support services.
The Homes First Case Manager will offer three years of flexible, trauma-informed, and culturally safe case management support to individuals and families in the program. This role is dedicated to helping clients achieve their housing goals and address their mental and physical health needs. The ultimate aim is to secure suitable and sustainable housing while supporting clients' recovery from personal challenges, thereby working to prevent future episodes of homelessness.
Selection Criteria:
A relevant tertiary qualification such as Social Work, Welfare or Behavioural Science and/or proven relevant experience to this role.
Knowledge and understanding of the Housing First Principles
Experience in providing holistic, strength-based, trauma informed case management support to vulnerable community members with multiple and complex needs.
An ability to adequately assess individual needs in a family context, and develop, evaluate, and update case support plans in relation to those needs using trauma informed practice.
An understanding of the issues that may lead to a family or individual accessing a homelessness service.
