We are looking for a Care Coordinator to join our expanding service delivery team in Dubbo, NSW (Wiradjuri Country). This exciting vacancy is ongoing, Full Time (75 hours per fortnight).

What will I do?
Assist in continuing implementation of a growing program that delivers care and services for people experiencing chronic disease and complex health issues
Provide care coordination, primarily via telehealth, and follow the client’s journey throughout their care
Support people with complex healthcare needs in effectively navigating the health system
Guide self-management and health literacy, empowerment and integration with general practice and key stakeholders.

What do I need?
Tertiary qualifications in an allied health discipline - nursing, social work, occupational therapy or other relevant profession
Registration with AHPRA or membership with the equivalent professional body
Ability to comprehensively conduct initial assessments and develop plans for care coordination
Understanding of the complexities and challenges involved in supporting individuals with chronic conditions, including mental health
A high standard of verbal, written, interpersonal and communication skills, including the ability to adapt services to meet varying needs
Ability to work with a diverse range of stakeholders to develop partnerships and service pathways.
