About the role
Coordinate service delivery for clients who need intensive and/or 24/7-hour supports to ensure ongoing service quality and support that is safe and appropriate, for clients and staff
This role is client retention and stakeholder focused – working with the client, their support network and intermediaries (funding Planners or Support Coordinators NDIS/TAC/WorkSafe)
Whilst IDS are not case managers, this role requires knowledge and experience of such a function in order to effectively liaise, escalate and engage with the professional care team, and intermediaries responsible for coordinating any care and/or funding requirements.
What you'll be doing
Onboard and maintain regular contact with new clients (online, email, phone or in the field) to ensure a smooth transition to IDS
Regular liaison with existing clients, their intermediaries and care team
Provide regular updates to Support Coordinators
Identify risks to clients and or services and escalate these to management and stakeholders accordingly
Manage client documentation and ensure care team are appropriately trained, in collaboration with HR
What we're looking for
2 – 3 years’ experience in casework or case management, preferably in disability, aged care, healthcare or out of home care, to plan, advocate and organise client care
Strong communication and interpersonal skills, with the ability to build rapport and trust with clients
Computer literacy with proficiency in the use of MS Outlook / Word and experience using a client management system or database
Excellent time management skills, with the ability to manage changing priorities
Understanding of NDIS in relation to client service planning
Essential Criteria
Must be eligible to Work in Australia
Must own or have regular access to a car
Current Working With Children's Check
Current NDIS Worker Screening Check.
