About the Role
Join our dynamic team as a Clinical Nurse within CALHN Integrated Care and make a meaningful impact on the lives of individuals with complex health needs. In this diverse and rewarding role, you will work collaboratively within a multidisciplinary team across clinic and community settings, including CALHN’s acute and sub-acute sites. Your focus will be on delivering culturally appropriate clinical interventions and assessments that support hospital avoidance, substitution, and reduced length of stay in the acute sector. You will play an integral role across services such as Hospital in the Home, Refugee Health Service, O’Brien Street Practice, and the Hospital Avoidance Supported Discharge Service, including its Outreach Team.
This position offers a unique opportunity to contribute to the health and wellbeing of vulnerable clients with complex needs through coordinated and integrated service responses. As a Clinical Nurse, you will bring a high level of professional practice, accepting accountability for your own nursing standards and for delegating tasks effectively. About You
We are seeking a passionate and experienced nursing professional to join our team. In this role, you will bring a combination of strong clinical expertise, leadership skills, and adaptability to thrive in a dynamic healthcare environment. To excel in this position, you will demonstrate:
- Qualifications and Registration: Current registration or eligibility for registration as a Nurse with the Nursing Board of Australia and a valid practising certificate.
- Skills and Abilities: Exceptional communication, clinical assessment, and problem-solving skills, with the ability to adapt to changing environments, manage competing priorities, and work both autonomously and collaboratively within multidisciplinary teams.
- Experience: At least 3 years of full-time equivalent post-registration experience, proven competence in relevant nursing practices, and experience in mentoring and leading less experienced nursing staff.
- Knowledge: A strong understanding of relevant legislation, codes, and standards, alongside quality improvement systems and contemporary healthcare issues within acute, sub-acute, and community settings.
