Collective Hope Community Services Ltd supports people with psychosocial disabilities in supported living accommodation. We provide NDIS participants with individual support needs with a range of supported accommodation options. With a team of qualified and dedicated staff throughout the Metropolitan areas of WA.

About the Role
We are looking for experienced Support Workers to join our team in Perth to support our participants in their independent living arrangements, helping them achieve their goals.
The aim of the NDIS is to give participants choice and control over what supports and services they need, how and by whom they are going to be provided.
Casual roles are available for this position.
The Duties of this role are:
Behaviour Support Plan implementation
Able and willing to work overnight shifts including weekends
Form and maintain good rapport and working relationships with participants to help them achieve their goals
Build participants’ capacity to live as independently as possible. This includes supporting participants with their daily living needs, personal care, physical health and well-being and accessing social and community participation
Selection Criteria
We are looking for someone with:
2+ years experience in mental health and disability support or other relevant fields
Significant experience with mental health and/or complex disabilities
Understanding of and experience with the NDIS practice guidelines and standards
Medication Administration competency
A professional commitment to making a difference in the lives of participants
A proven capacity to build rapport with participants from a diverse range of backgrounds with the ability to listen, engage and empower participants to achieve their goals in a structured way
A positive demeanour and resilience in working with challenges
Strong alignment to the Collective Hope mission, vision and values

NDIS Worker Orientation Module Certificate
NDIS New Worker Induction Module Certificate
Minimum Cert III in Mental Health/Disability/Community Services (or other relevant fields)
Current Driver’s Licence and a reliable vehicle (with comprehensive insurance)
First Aid Certificate
Medication Certificate
Infection Control Certificate
Food Safety Certificate
Covid-19 Vaccination Status
