Are you a self-motivated and compassionate individual with a passion for making a genuine difference in people's lives? We have an outstanding opportunity for an experienced Care Partner to join a respected non-profit community home care provider in Brisbane.
About the Organisation:
For 25 years, our client has been a pillar of support in South East Queensland, delivering quality in-home services. They are a leading provider of HCP, CHSP, DVA , ACVVS, and Transition Care services, experiencing exciting growth.
About the Role:
As a Care Partner, you'll play a crucial role supporting HCP clients with transition to the new Support at Home program. You will also support CHSP clients. This will include onboarding new clients and providing ongoing care management, care planning and monitoring to ensure a high-quality service.
You will be passionate about empowering clients to remain living independently in their own homes.
Ideally, you'll bring experience in both CHSP and HCP and have knowledge of the new Support at Home program. You'll have a manageable caseload in the Brisbane North area and thrive on building strong relationships with your clients and their families. You'll benefit from excellent organisational support, working closely with a collaborative team of other Care Partners, Registered Nurses, and Allied Health professionals.
This is a permanent, full-time, office-based role located in Spring Hill. Company vehicles are available for client visits.

What You'll Bring
Demonstrated experience and knowledge of either Home Care Packages or CHSP programs
Some knowledge of the upcoming Support at Home program (training will be provided)
Strong organisational skills including problem solving, prioritisation and multitasking
Excellent communication and interpersonal skills with a collaborative team spirit and "can do" attitude
Proficiency in MS Office and care management databases
Current open Australian Drivers Licence
Current First Aid and CPR certification
You will also need to be a permanent Australian resident and have a National Police Check (or willingness to obtain).
Ideally you will fully vaccinated - COVID19 and Influenza.
