Lifest HealthCare is seeking a compassionate, experienced, and highly organised Team Leader to join our dedicated team supporting individuals living with mental health and physical disabilities. This is a rewarding opportunity to lead a team of Disability Home Care Workers while making a meaningful impact in the lives of our participants. This is a hands-on role where you'll be part of the care team.
As a Team Leader, you will:
Oversee the day-to-day operations of Supported Independent Living homes - Monday to Friday
Provide direct care and support to participants with varying diagnoses
Lead and mentor a team of Disability Support Workers
Manage rosters, appointments, documentation, and reporting
Ensure high-quality, person-centred care aligned with NDIS standards
We’re seeking someone who is:
Passionate about empowering people with disabilities
Experienced in a similar leadership role within the disability sector
A confident communicator with strong organisational and time management skills
Skilled in team management, delegation, and problem-solving
Knowledgeable in NDIS standards, legislation, and best practices
Key Responsibilities
Participant Support
Update and manage individual care plans
Provide hands-on support and community access
Liaise with families, allied health professionals, and stakeholders
Complete documentation including shift notes, incident reports, and progress updates
Leadership & Administration
Mentor and coach team members
Review and action shift notes
Manage rosters, leave, and team availability
Ensure compliance with reporting and documentation standards
Attend internal and external meetings as required
Staff Management
Lead the training of new staff
Conduct buddy shifts
Promote staff wellbeing and a positive team culture
Monitor workloads to prevent burnout and ensure quality care
Essential Requirements
Certificate III or IV in Disability (or working towards within 12 months)
Minimum 3 years’ experience in a care-related role
Previous experience in a Team Leader or Supervisor role ESSENTIAL
Strong understanding of disability support and mental health
Valid driver’s licence and reliable vehicle
Current First Aid & CPR (or willingness to obtain)
Paid NDIS Worker Screening Card
Full working rights with Monday to Friday daytime availability - this role is not suitable for student visa holders
