About this role
As a Care Partner, you’ll be the vital link between our clients and the new Support at Home program—ensuring older Australians receive the right care, at the right time, in the way that best suits them.
This is a dynamic, hands-on leadership role where you’ll guide a team of passionate Home Care Workers to deliver high-quality, person-centred care that upholds dignity, independence, and quality of life. You’ll work closely with clients, their families, and external providers to tailor support and foster strong, trusted relationships.
Key responsibilities
- Assess Client Needs: Conduct initial and ongoing assessments to understand physical, emotional, and social needs, identifying risks and tailoring care plans with clients and families.
- Care Planning: Develop, review, and update person-centred care plans aligned with client goals and Support at Home funding.
- Coordinate Services: Organise in-home supports and liaise with external providers to ensure timely, quality care.
- Advocate and Educate: Support clients to understand their funding, choices, and rights, and advocate for access to appropriate services.
- Budget Oversight: Monitor individual budgets and guide clients on effective use of funds.
- Relationship Building: Build strong connections with clients, carers, and providers, supporting smooth transitions and ongoing care.
- Compliance and Safety: Maintain accurate documentation, uphold aged care standards, and proactively manage risks.

What you can bring to the team
You’re a natural people leader with a heart for community care and a talent for getting the best out of your team. You’re confident navigating the aged care space and thrive in a fast-paced, mobile work environment.
To be successful, you’ll need:
- Cert III in Individual Support (Ageing) or equivalent
- Experience leading a team in aged or community care
- Working knowledge of aged care standards, funding, and compliance
- Current First Aid and CPR certification
- A reliable, comprehensively insured vehicle and a valid driver’s licence
- Strong communication, organisation and problem-solving skills
Desirable:
- Sector experience in home care or aged care
- Customer service or sales experience desirable but not essential
- A positive can-do attitude
- Intermediate computer skills (Word, Excel, Outlook)
Possess or willingness to obtain:
- A current National Police Certificate
- Up-to-date First Aid and CPR Certification
- Undergo a National Personal Insolvency Check.
