Join Karuna as a Carer Services Navigator and play a vital role in helping unpaid carers access the support they need. This is a frontline, relationship-driven position where your empathy and initiative can make a meaningful difference in people’s lives - often at their most vulnerable.
As a Carer Services Navigator, you’ll support carers from intake through to planning and referral - connecting them with tailored supports across health, community, and home environments. You’ll advocate, assess risk, coordinate services, and promote the inclusion and wellbeing of carers in every setting you work in.
What You’ll Be Doing:
Conduct carer assessments using tools such as the Carer Star
Develop personalised action plans based on carer goals and wellbeing
Provide timely referrals to internal and external supports
Work collaboratively with providers (e.g. ACAT, hospital discharge teams)
Maintain accurate, confidential records in client management systems
Advocate for carers within community and health networks
Promote carer inclusion, awareness, and equity
Represent Karuna in service networks and local forums
What You’ll Bring:
Essential:
Qualification in community services, health, or social support (Cert IV or higher)
Knowledge of carer needs and the impact of the caring role
Experience in intake, assessment, risk planning, and person-centred support
Valid Driver’s Licence, National Police Check and Blue/Yellow Cards
Legal right to work in Australia
Desirable:
Knowledge of the Carer Gateway program and local supports
Familiarity with trauma-informed and culturally safe practice
Experience with client management systems
Key Attributes:
Warm, calm, and empathetic communicator
Organised and proactive with strong time management skills
Able to work autonomously and across community settings
Committed to inclusive practice, continuous learning, and team collaboration
Digitally capable (Microsoft Office, databases, communication platforms)
