Job Status: Up to 0.6 FTE (flexible FTE available)
Pay (per annum): $37,525 - $40,615 (pro rata + super + leave loading + salary packaging)

About the Role
We are seeking an Aboriginal Outreach Worker to join our Closing the Gap Riverland Team for the Integrated Team Care (ITC) Program. The successful applicant will be someone with a passion to provide support to Riverland Aboriginal and Torres Strait Islander community members and their families, in accessing primary health care services.
The ITC Program
The aims of the ITC Program are to:
· Contribute to improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through access to care coordination, multidisciplinary care, and support for self-management; and
· Improve access to culturally appropriate mainstream primary care services (including but not limited to general practice, allied health and specialists) for Aboriginal and Torres Strait Islander people.

The successful applicant will be remunerated in accordance with FocusOne Health’s Enterprise Bargaining Agreement (EBA) dependent on skills, experience, and qualifications.
