About the Role: Port Lincoln Aboriginal Health is seeking a dedicated and compassionate Aboriginal Community Case Worker to join our team on a full-time, permanent basis. This role is pivotal in providing comprehensive case management and support to our Aboriginal community, ensuring culturally appropriate and holistic care.

Key Responsibilities:

Case Work:

  • Provide coordinated case management for clients
  • Promote early detection and intervention for mental health, substance use, and SEWB (Social and Emotional Wellbeing) issues
  • Deliver culturally appropriate health and SEWB education programs
  • Collaborate with other health care professionals for a multi-disciplinary approach
  • Maintain information systems and records for efficient client management
  • Participate in quality assurance programs
  • Perform other relevant duties as directed

Drug and Alcohol:

  • Conduct assessments to identify substance use patterns
  • Develop and implement individualized treatment plans
  • Provide counselling and support for drug and alcohol dependency
  • Facilitate group programs on substance abuse recovery and relapse prevention
  • Coordinate care with healthcare professionals for holistic treatment
  • Educate clients and families about substance abuse and treatment options
  • Monitor progress and adjust treatment plans as needed
  • Advocate for clients and connect them with community resources

Essential Qualifications, Knowledge and Skills:

  • Have or be willing to obtain competencies and qualifications as an Aboriginal Health Worker by completing a nationally accredited Certificate
    III or IV in Aboriginal and/or Torres Strait Islander Primary Health Care, or other relevant qualification.
  • Competencies necessary to practise in the particular practice setting
  • Willingness to undertake further education, training and other professional development activities relevant to the position
  • Must be able to use own initiative
  • Must be able to communicate effectively with professionals from other disciplines
  • Must be able to communicate effectively with both Aboriginal and Non-Aboriginal people
  • Ability to work effectively in a multi-disciplinary team approach
  • Ability and willingness to approach with sound work ethics and reliability
  • Demonstrated ability in working independently with minimal supervision, determining priorities, meeting deadlines, planning and organising work
  • Ability to observe strict confidentiality regarding all information pertaining to all client/s of the Organisation and organisational matters
  • Demonstrated ability to effectively, accurately and appropriately utilise information technology, in particular Microsoft Office products and data entry programs.
  • Experience in working with Aboriginal people.
  • Experience in working in rural/remote communities.
  • Experience working with clients with drug and alcohol dependence.
