Salary: From $70,000 – $100,000 per annum pro rata (dependent on qualifications and experience)

We are now seeking a qualified Aboriginal Health Practitioner (AHP) to join our dynamic and supportive clinical team.
About the Role
As an AHP, you will work collaboratively with general practitioners, nurses, and allied health professionals to provide holistic, culturally responsive care to Aboriginal and Torres Strait Islander patients and the broader Palmerston community. Your role will span both GP and urgent care settings, with a strong focus on prevention, health education, chronic disease management, and clinical support.
Key Responsibilities
Deliver culturally appropriate clinical care and health education to patients.
Undertake health assessments, chronic disease screening, immunisations, wound care, and follow-up support.
Support patients and families with care navigation and coordination.
Collaborate in multidisciplinary care teams, contributing to case planning and review.
Participate in health promotion, outreach, and community engagement activities.
Maintain accurate clinical documentation and comply with AHPRA scope of practice.
Contribute to cultural safety across the clinics through advocacy, staff education, and cultural liaison.
Selection Criteria
Essential:
Current registration as an Aboriginal Health Practitioner with AHPRA.
Completed HLT40221 Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice.
Demonstrated understanding of primary health care, health promotion, and chronic disease management.
Strong communication and interpersonal skills, including the ability to engage effectively with Aboriginal and Torres Strait Islander clients and communities.
Commitment to culturally safe care and closing the gap in health outcomes.
Desirable:
Experience working in general practice or urgent care settings.
NT Immunisation endorsement.
Knowledge of Best Practice software or similar EMR systems.
Experience with outreach, health screening, or community health programs.
