About the role:

The role of the Home Care Registered Nurse is to provide clinical assessment and clinical leadership to Home Care Package and/or Residential Aged Care clients in Greater Sydney area.

This is a remotely based role, and you will be required to travel within Sydney.

KEY ROLES AND RESPONSIBILITIES

Clinical Assessment & Care Planning: Conduct assessments, contribute to support plan development and reviews, and participate in case conferences to ensure responsive, person-centred care.

Clinical Oversight & Supervision: Provide clinical leadership to staff, supervise care delivery, and ensure best practice in wound care, medication (if applicable), and overall consumer wellbeing.

Incident & Health Management: Manage and respond to incidents, monitor changes in consumer health, and coordinate medical and allied health reviews as needed.

Communication & Collaboration: Maintain clear, timely communication with consumers, families, guardians, and staff, fostering effective team collaboration and consumer-focused care.

Documentation & Compliance: Accurately document care activities and health changes, ensuring timely, compliant records that meet clinical and organisational standards.

Essential Attributes & Requirements

Professional Credentials & Compliance:

Current AHPRA registration.

Current police check.

Driver’s licence with access to a safe and reliable vehicle.

Ability to identify and manage early health changes to prevent deterioration or hospital admissions, with strong knowledge of Clinical Governance and Aged Care Quality Standards.

Excellent written and verbal communication skills, professional presentation, and computer literacy.

Dedication to high-quality aged care and sensitivity to cultural diversity in care delivery.
