Support Worker Position Description (but not limited to):
Support NDIS participants to engage in activities and tasks, including capacity building, in-home support, and community access within the South Eastern and Eastern areas of Victoria
Support NDIS participants to access the community.
Monitor the safety and well-being of the participant and report any concerns to the Team Lead.
Ensure completion of appropriate documentation such as participant progress notes, incident reports, and goal attainment using ShiftCare and the Client Management System.
Undertake relevant participant-specific and mandatory training modules to maintain NDIS practice competency requirements.
Actively contribute to a team environment through open communication, participation in regular team meetings, planning processes, and professional development /supervision.
Provide backup and support to other staff, as necessary.
Report any quality and safety incidents or hazards at work immediately to the Team Lead.
Carry out their roles and responsibilities as detailed in the relevant policies and processes.
Qualifications – preferred
Cert IV Child Protection, Mental Health, Disability, Community Services, or similar or relevant tertiary qualification or experience in the field.
Current National Police Check.
Working with Children Check.
NDIS Workers Orientation (or willing to complete).
Current Driver's license and fully comprehensive insurance.
Competencies and experience:
Experience working in the disability field.
Experience working with people with disability who have been involved in the criminal justice system, and behavior of concern (preferred).
Genuine passion for the field of work.
Strong engagement, enthusiasm, resilience, and self-care.
Experience in medication administration or willingness to be trained.
Understanding of Behaviour Support implementation and restrictive practice (mandatory).
