About the Role
As a Home Care Clinical Coordinator, you will play a key role in coordinating and delivering high-quality, person-centred care and services to clients receiving a Home Care Package. You will work closely with clients, families, and healthcare professionals, ensuring optimal health and wellbeing outcomes for our clients. This position also supports the Home Care Management team in areas including gerontology, palliative care, dementia support, and mental health.
Key Responsibilities
- Case management, care planning, and ongoing assessment of clients’ needs
- Develop personalised care plans and client budgets that maximise independence and choice
- Provide clinical guidance in specialist areas such as dementia, palliative care, and hospital-to-home transitions
- Liaise with families, healthcare professionals, and service providers to ensure holistic support
- Monitor, support, and guide Home Care Personal Carers in delivering safe, effective care
- Ensure accurate record-keeping and adherence to legislative and organisational guidelines
- Contribute to continuous improvement initiatives and quality systems
Mandatory Qualifications & Requirements
- Registered Nurse (RN) with current AHPRA registration
- Valid driver’s licence
- Police Check and NDIS Worker Screening Clearance (or willingness to obtain)
Essential Skills
- Strong organisational and problem-solving skills
- Clinical expertise in aged care, dementia, and behaviour management
- Excellent communication, negotiation, and relationship-building abilities
- Proficient in Microsoft Office suite
- Self-motivated and a strong team player
Desirable
- Additional qualifications in aged care, dementia, or community nursing
- Experience working collaboratively with families, guardians, and medical professionals
