The Role:
We are expanding and have new participants ready to receive supports, Heal At Home is seeking multiple roles Registered Nurse / Clinical Nurse to join our compassionate team. These are casual roles, offering flexibility to work. In this role, you will be responsible for delivering high-quality care to clients in the home setting, assessing their health needs, managing complex conditions, and supporting them with holistic care approaches.
Key Responsibilities:
Provide nursing assessments and comprehensive care
Manage and monitor medications, complex wound care, continence support, and stoma care
Offer specialised care for clients with conditions such as diabetes, epilepsy, and complex bowel care
Develop and review individualised care plans in collaboration with stakeholders and other healthcare professionals.
Essential Qualifications and Skills:
Registered Nurse with a valid AHPRA registration
Nursing experience, preferably in home or community-based care
Experience with complex care management (ventilator care, tracheostomy, epilepsy, etc.)
Strong communication and interpersonal skills
Ability to work autonomously and as part of a team
A genuine passion for supporting clients
Willingness to travel

To be considered for this role, you must provide the following:
Current AHPRA registration without conditions
Current and valid National Police Check (Issued within last 12 months)
Current full WA driver's licence
Valid NDIS Worker Screening Check
Valid Working with Children Check
Right to work in Australia
Assist Clients with Medication certificate
Current First Aid certificate
Current CPR Training certificate
Manual Handling certificate
