Your Aged Care at Home is a registered charity and public benevolent institution that is approved to provide both the Home Care Packages Program and NDIS services.

About the role

This role is open to applicants keen to work with our community of aged and persons with disabilities, providing in-home physiotherapy, including assessments, treatment, care plans and reports.

You will be joining a growing business that is client and community focused.

Key Responsibilities

Focus on client needs, anticipate and identify client needs and take action to meet these needs.

Develop goals and best practice strategies with clients to promote health and optimum function.

Work in a team environment supporting our Registered Nurse and Coordinators.

Maintain a high standard in providing prompt and solution orientated service to clients.

Cultural competency when dealing with clients and staff.

Maintain confidentiality of information at all times.

Exhibit a concern for excellence in service delivery to older people and persons with a disability.

Demonstrated knowledge of the Home Care Packages Program and / or NDIS Supports is desirable, but not essential.

Conform to regulatory requirements for Aged Care and NDIS.

Skills and experience

Degree qualification in Physiotherapy with AHPRA registration.

Experience in physiotherapy treatment of adult/elderly clients (minimal of 1 year).

Current valid First Aid and CPR certification (or willing to apply).

Driver Licence.

Prior experience in the aged care or disability sector is desirable, but not essential.

Excellent verbal and written communication skills.

Excellent documentation skills.

Computer literate and competent in the use of various apps, including Outlook and Customer Relationship Management (CRM) software.

Excellent communication and phone skills with the ability to liaise and consult with a wide range of clients, health professionals and team members.

NDISWC Clearance and Police Check will be required for the role.
