We are currently seeking a passionate and dedicated Care Coordinator to join our dynamic team, working closely with our clients and front-line support workers to deliver outstanding care.
The Role:
As a Care Coordinator, you will be responsible for managing and growing the client base within the Care Unit, overseeing the development and review of client care plans, and ensuring the delivery of high-quality support. You will supervise a team of support workers, liaise with clients & stakeholders, and maintain Service Agreements and budgets to ensure all needs are met. Key Responsibilities include:
- Client Relationship Management: Grow and maintain the client base, ensuring long-term, positive relationships with clients and their families.
- Care Plan Development: Develop, manage, and review client care plans to meet individual needs, goals, and objectives.
- Customer Service & Support: Provide ongoing support to clients, ensuring they receive high-quality service to help them achieve their goals.
- Team Supervision: Lead and supervise a team of front-line support workers, ensuring the delivery of reliable, high-quality care.
- Service Agreements & Budgets: Develop, review, and maintain Service Agreements and Quotes in line with client funding and budgets.
- Client Onboarding & Exit: Ensure all onboarding and exit procedures are carried out accurately and in compliance with business policies.
About You:
- Relevant qualifications in Disability, Community Services, Aged Care or a related field
- Demonstrated experience in a similar Care Coordinator or Case Management role (within the disability sector is highly desirable).
- Excellent organisational and leadership skills, with the ability to manage and supervise a team effectively.
- Strong communication skills that display a commitment to care and empathy for clients and staff
- Ability to develop, manage, and review client care plans and Service Agreements in line with budgets and client needs.
- Exceptional organisational and time managements along with attention to detail
- A thorough understanding of relevant legislation, policies, and procedures related to the disability sector.
- Valid and unrestricted Drivers Licence
- Current Working with Children Check and NDIS Worker Screening Clearance
- First Aid & CPR certificates (or willingness to obtain)
