About the Role

As our Care Coordinator, you’ll play a vital role in leading support workers, ensuring seamless onboarding, mentoring our Administrative Officer, managing participant experiences, resolving conflicts, and supporting business growth. You’ll champion our core values — teamwork, excellence, respect, creativity, empowerment, efficiency, and inclusivity — while maintaining NDIS compliance and fostering a supportive team culture.

What You’ll Do

Support Worker Management: Onboard, roster, and mentor support workers, ensuring retention and satisfaction.

Admin Mentorship: Guide and support the Administrative Officer in day-to-day operations and task prioritization.

Participant Onboarding & Support: Facilitate smooth participant intake, create tailored support plans, and maintain regular check-ins.

Compliance & Reporting: Handle incident investigations, NDIS reporting, and ensure timely communication.

Stakeholder Engagement: Conduct meetings with participants, families, and staff, and follow through on outcomes.

On-Call Support: Participate in a rotating schedule for emergency response.

SIL Coordination: Work closely with SIL Team Leaders to manage staffing, care planning, and house visits.

Growth & Outreach: Support business development through community outreach and participant sign-ups.
