As our organisation grows, we are seeking a permanent, full-time Care Services Coordinator to join our committed team and play a central role in supporting participants to thrive.
About the Role
As a Care Services Coordinator, you’ll be the key point of contact for a portfolio of clients, ensuring that their care is person-centred, well-coordinated, and delivered with compassion and consistency.
This role involves a balance of client contact, care planning, coordination, and team leadership. You’ll work closely with our Registered Nurses, Support Workers, and external stakeholders to ensure quality outcomes across the board. The position is based at our Brendale office with regular local travel and optional flexibility to work from home when appropriate.
Key Responsibilities
Undertake person-centred assessments, support planning, and case coordination
Write Support Plans and Service Agreements that reflect client goals and needs
Monitor service delivery and resolve issues as they arise
Build trusted relationships with participants, families, and external providers
Provide guidance, mentoring, and supervision to allocated Support Workers
Collaborate with our Roster and Clinical teams to ensure seamless service
Conduct client visits, reviews, and handle risk reporting as needed
What You’ll Bring
✔ Strong understanding of the NDIS, Home Care Packages, and CHSP frameworks
✔ Experience in care coordination, client services, or case management
✔ Excellent communication, empathy, and problem-solving skills
✔ Proven ability to write clear, professional care documentation
✔ Passion for supporting clients to achieve their goals and live well
✔ Ability to manage competing priorities with attention to detail
✔ High-level computer literacy and confidence in using client management systems
Essential Criteria
Certificate IV or Diploma in Disability, Aged Care, Individual Support, or Community Services
Current First Aid & CPR certification
NDIS Worker Screening Check (Yellow Card) & Blue Card
National Police Check (within last 12 months)
Valid driver’s licence + reliable, insured vehicle
Understanding of and commitment to the NDIS Code of Conduct
Studying toward or holding a nursing qualification (e.g., EEN or RN) will be highly regarded.
