About Beacon Support

At Beacon Support, we deliver in-home and community-based support to people living with disability, complex care needs, and age-related conditions across Southeast Queensland. The work we do every day has a lasting impact. We don’t just provide services — we enable lives. It’s about empowering our clients to live with dignity, purpose, and choice.

What sets us apart is not only what we do, but how we do it. We are a team, united under the Beacon Support banner, and it’s the unique strengths each of us brings — our compassion, diversity, professionalism, empathy, diligence, and care — that creates something truly special.

The Beacon Way is how we show up for each other, for our clients, and for our community. It's reflected in our actions, our standards, and our interactions. It defines our culture and represents what we stand for: people-first support, unwavering integrity, and the belief that everyone deserves to live their best life.

As our organisation grows, we are seeking a permanent, full-time Care Services Coordinator to join our committed team and play a central role in supporting participants to thrive.

About the Role

As a Care Services Coordinator, you’ll be the key point of contact for a portfolio of clients, ensuring that their care is person-centred, well-coordinated, and delivered with compassion and consistency.

This role involves a balance of client contact, care planning, coordination, and team leadership. You’ll work closely with our Registered Nurses, Support Workers, and external stakeholders to ensure quality outcomes across the board. The position is based at our Brendale office with regular local travel and optional flexibility to work from home when appropriate.

Write Support Plans and Service Agreements that reflect client goals and needs

Monitor service delivery and resolve issues as they arise

Build trusted relationships with participants, families, and external providers

Provide guidance, mentoring, and supervision to allocated Support Workers

Collaborate with our Roster and Clinical teams to ensure seamless service

Conduct client visits, reviews, and handle risk reporting as needed

What You’ll Bring

✔ Strong understanding of the NDIS, Home Care Packages, and CHSP frameworks
✔ Experience in care coordination, client services, or case management
✔ Excellent communication, empathy, and problem-solving skills
✔ Proven ability to write clear, professional care documentation
✔ Passion for supporting clients to achieve their goals and live well
✔ Ability to manage competing priorities with attention to detail
✔ High-level computer literacy and confidence in using client management systems

Essential Criteria

Certificate IV or Diploma in Disability, Aged Care, Individual Support, or Community Services

Current First Aid & CPR certification

NDIS Worker Screening Check (Yellow Card) & Blue Card

National Police Check (within last 12 months)

Valid driver’s licence + reliable, insured vehicle

Understanding of and commitment to the NDIS Code of Conduct

Studying toward or holding a nursing qualification (e.g., EEN or RN) will be highly regarded.
