About the Role – Make a Real Difference Every Day
We’re looking for a passionate Family Support Practitioner to join our team to support children and families.
In this role, you’ll provide outreach-based, family-centred support to families who are facing complex challenges. You’ll work directly with children, parents, and carers to build trust, understand their needs, and create tailored case plans that help keep kids safe and families strong.
You’ll also coordinate referrals to other services, run group programs, and collaborate closely with professionals and community partners to support long-term positive outcomes.
- Level 4 SCHCADS Award.
- Salary range: $85,127 - $91,588 + Superannuation.
- Full time position government funded until 30 June 2030 with possible extension.
What You’ll Be Doing
- Provide case management and support to families in their homes or other community spaces.
- Create and lead case plans that address both child protection concerns and the family’s unique needs.
- Connect families with services and long-term supports by building strong referral pathway.
- Work closely with professionals like Child Safety and schools to support better outcomes for families.

What You’ll Bring
- A degree in social work, allied health, human services, or a related field
- At least 2 years’ experience supporting families and children, ideally within a child protection framework
- Great communication skills, plus cultural awareness and the ability to work with families from diverse backgrounds.
- An ability to work collaboratively within a team and alongside other services to support children.
Other Things You’ll Need
- A current Working with Children Check (or eligible for professional exemption).
- A valid driver’s licence.
- The legal right to work in Australia (proof of eligibility required).
