About the Position

We are looking to welcome a Family Support Practitioner to deliver an outreach model of complex case-management programs to families, predominantly within their home environment.

• Full time opportunity.
• Government funded to 26 September 2026 with possible extension.
• Level 4 under the SCHCADS Award. Salary range is $85,127 – $91,588 + Super.

Key Responsibilities:

• Maintain a caseload of families and provide evidence-informed case management interventions for clients with complex needs.
• Engage with families, obtain consent, and develop a case plan in conjunction with the family and which addresses unmet needs and child protection concerns.
• Facilitate social learning, community connectedness, good health, and well-being.

About You

• A tertiary degree in a relevant discipline (allied health, social work, human services).
• Two years’ experience providing case-management and therapeutic services to families, caregivers, young people, and children in a child protection framework is preferred.
• We are also seeking new graduates wanting to develop a case management framework and associated skills, in intensive family support. Graduate applicants are strongly encouraged to apply.
• Cultural awareness and an understanding of how to communicate and work effectively with families from different socio-economic and cultural backgrounds, including Aboriginal and Torres Strait Islander people.

• Eligibility for a Working with Children Check or professional exemption.
• Current driver’s licence.
