About the role:

St Anthony Family Medical Practice are seeking a motivated Permanent Part-Time Practice Nurse to join our dedicated team in Rochester and Elmore. This role involves working closely with GPs and other healthcare workers to assist in delivering high level primary care to our patients.

Responsibilities:

Health assessments and chronic disease management plans

Vaccination administration

Cold chain management and infection control

Patient triage, wound management and assisting with minor procedures

Managing recall and reminder system; clinical data management; accreditation compliance

Essential criteria:

Proven multi-tasking, problem solving and time management skills

A caring and collaborative approach to patient care

Current AHPRA registration as a Registered Nurse

Working with Children Check

Current CPR certificate

Desirable but not essential criteria:

Experience using Best Practice software

Recent acute/practice nurse experience

Experience in chronic disease management
