About the role
Role: Support Coordinator / Service Leader
Our Seed Team is currently seeking a team member for our Support Coordination team in our Sunshine Coast group. As a small team, the role will be multifaceted, covering multiple areas of the business.
In this role, you will work closely with our team, participants, their families, and other service providers to develop and implement personalised support plans. Your expertise will be crucial in guiding participants through available resources and ensuring they receive the services they need.
Scope:
Partner with NDIS participants and their families to provide individual Support Coordination services.
Assist participants in understanding and implementing their NDIS plans to enhance their capacity and independence
Review support arrangements to gain insights into each client and create personalised care plans
Assess and work within each participants funding level, helping them access additional funding if required
Manage plan review processes, including unscheduled reviews and internal complaints, to ensure quality outcomes
Prepare for potential crises and advocate for changes that enhance participant safety and well-being
Foster effective working relationships and maintain open communication with participants, caregivers, families, external services, and other key stakeholders
Build and sustain strong relationships throughout the sector to ensure participants have choice and control
Commit to best practices and person-centered outcomes across the sector
Promote a culture of collaboration, respect and continuous improvement
Provide team leadership including communication, scope management, HR and recruitment.
About you
To be successful in this role we are looking for someone who has the following:
Experience in Support Coordination
Leadership experience
Knowledge of practice and guidelines as they relate to the NDIS
Working with Children Blue card
NDIS Worker Screening
Driver’s license and a reliable car
First Aid and CPR Certificates
Tertiary qualifications desirable but not essential: Psychology, Social Work, Occupational Therapy or other Allied Health disciplines
Certificate IV in Disability, mental health or other qualification relevant to the field of human services with relevant experience.

Track record of successfully meeting key performance indicators (KPIs).
Superior written and verbal communication skills.
Excellent interpersonal skills, with the ability to engage with participants in a professional and empathetic way.
Experience or demonstrated competency in support planning and case management.
Established ability to network and foster positive relationships with local service providers.
