About the Role
We are currently seeking a Service Delivery Officer to join our team in Cranbourne, VIC. This vital position oversees a portfolio of NDIS participants and staff of key workers. You will play a central role in ensuring the delivery of person-centered, high-quality services that meet participant goals while upholding all safeguarding and compliance obligations.
Key Responsibilities
Maintain regular communication with participants, support workers, and stakeholders to ensure continuity and quality of care.
Conduct quarterly 1:1 sessions with support staff to strengthen team engagement and development.
Coordinate and adjust rosters based on participant needs, staff availability, and service continuity.
Monitor and ensure the accuracy of participant documentation, including service plans, risk assessments, and progress notes.
Collaborate across internal teams and external providers to promote integrated and person-centred service delivery.
About You
Relevant qualifications in community services, mental health, disability, or aged care (minimum Certificate IV or higher).
Demonstrated experience in service coordination, support planning, or frontline disability/mental health services.
Knowledge of the NDIS Practice Standards, Code of Conduct, and participant safeguarding principles.
Strong communication and interpersonal skills, with a solution-focused approach.
Proficiency in case documentation, scheduling systems, and Microsoft Office.
Flexibility to work across weekdays with some availability for after-hours support as required.
Mandatory Requirements
Current NDIS Worker Screening Check (or willing to obtain)
Current Working with Children Check - Employee Level (Victoria)
National Police Clearance (within 12 months)
Completion of the NDIS Worker Orientation Module
