NT Integrated Therapy are seeking an exceptional NDIS Support Coordinator to join our team providing NDIS service delivery across the Top End. Working in our Humpty Doo office location Monday to Friday full time, you will work in a team of coordinators striving to deliver only the best of services to a vast portfolio of wonderful clients.

This position has the potential for remote travel at various intervals throughout the year, in partnership with other organisations and stakeholders in delivering NDIS services to remote and very remote located participants.

This position will commence under a probation period as outlined below, with the intention to progress to a permanent position for the right applicant.

Experience working in an NDIS Support Coordinator role or in an NDIS service delivery role highly desirable but not essential.

Your responsibilities as an NDIS Support Coordinator are, but are not limited to:

- Connecting with NDIS participants ongoingly and supporting them to establish, engage and maintain supports that will enable them to live a life full of choice and control.

- Closely monitoring and reporting on plan budgets and overall plan usage, outcomes and participant benefit.

- Working with/for the participant to schedule supports within their budget and in-line with their NDIS goals.

- Provide referral and connectivity with related service providers continuously.

- Work closely with participants, providing ongoing and flexible face to face engagement with them and their stakeholders.

- Maintain communication and build positive relationships with various NDIS providers and NDIS planners, providing evidence based feedback on the progress of participant plan supports and services to ensure the best outcomes for the client.

A little about the exceptional person we are seeking to fill this position:

- Desirable minimum Cert III qualification in Community Services or a related field.

- You hold a valid Drivers License and you have your own reliable vehicle.

- You hold both a National Police Clearance Check and Working with Children's Clearance OCHRE Card, or have ability to obtain prior to the commencement of your employment.

- You are trustworthy, reliable, flexible, able to work autonomously and under supervision. You have a natural ability to flourish in a high demand environment and can think outside of the square. You are able to take initiative, be confident and knowledgeable within your position, constantly seek self-accessible professional growth and knowledge around the ever-changing NDIS, all while providing services in-line with the NDIS Act.

- You have lived or working experience in disability, mental health, nursing or education with a keen interest in enriching the lives of those living with a disability and their loved ones.

- You have ability to be in the office during business hours, Monday to Friday 8.00 – 4.30.

- Lastly, you have a "can do" attitude. You are a team player with a glass half full outlook and an ability and eagerness to make a real positive difference to the lives of participants and their families.

Contract length: 6 months probation as full time permanent for the right candidate, with position permanency offered after successful completion of the probation period.
