Future Wellbeing is a values driven NDIS registered disability service provider based in Sydney South West. Our team is growing and we are seeking an ethical and experienced support coordinator to join our close knit team. This is a hybrid/ remote role for a NDIS Support Coordinator, with some face to face meetings as required within the Sydney Metro. The Support Coordinator will be responsible for managing and coordinating support services for clients, assessing client needs, managing budgets, developing care plans, and collaborating with healthcare providers in accordance with NDIS frameworks.

The role also involves providing guidance and support to clients and their families, maintaining accurate records, and ensuring the delivery of high-quality care. You will support a variety of participants from different cultural backgrounds with different disabilities including mental health diagnosis.

Experience as a support coordinator is essential with a minimum 12 months in delivering support coordination services. You will be supported by experienced managers who want to see you flourish, embrace and succeed in your role.

Skills and Qualifications

Excellent communication and interpersonal skills

Ability to multitask and prioritise tasks effectively

At least 2 years experience within the disability sector

Knowledge of NDIS including restrictive practices & behaviour supports

Flexible approach to your work & be able to work with a variety of people

Strong organisational skills and attention to detail

Empathy and compassion towards clients and their families

Knowledge of other government agencies and healthcare

Previous experience in a similar role is a plus

Ability to work independently and as part of a team

Lived Experience is highly regarded

Proficiency in Microsoft Office and client management software

Mandatory Requirements

Minimum Cert 4 in a related field

Own reliable vehicle, license & relevant insurances
