About Circle Specialist Coordination

Circle SC is an independent boutique NDIS registered company providing high quality NDIS Specialist Support Coordination to adults and children with complex support needs.

Circle SC was started to solve a problem in our community: the NDIS is too complicated. Our mission is simple. We aim to simplify the NDIS. We cut through complexity, take action and get results.

The people we support often experience the biggest barriers to engaging with services and are most in need of our support. This could be people living with disabilities that are at risk of becoming homeless, living with neurological conditions, interfacing with the justice system, children at risk, people expressing their unmet needs through ‘behaviours of concern’, people coming in or out of the hospital system, and Aboriginal and Torres Strait Islander people.

As a provider of Specialist Support Coordination we aim to overcome complex barriers and navigate people through complex systems in order to live a good life. We focus on building strong relationships through compassion, empathy and being led by the person themselves.

Circle SC is a small person-centred and values driven company that is well supported and has well developed systems in place. Currently the company is a team of six, Director Dan McCutcheon, 4x specialist support coordinators Emma, Jayde, Tess and Josh and our social media wizard El.

We have some of the smallest caseloads in the sector with each coordinator supporting 15-20 participants.

This is an exciting opportunity to not only provide Specialist Support Coordination but also to positively influence the strategic direction of a fast-growing new business and inspire us to be the best we can possibly be.

The newest Circle SC team member will work from home with each Friday an 'in-office day' in a co-working space. You will visit participants and families throughout the week around Perth Metro.

There will be a 6-8 week period of onboarding where you will work with the Director Dan in a co-working space in Mosman Park to learn the ropes and gradually build up your caseload.

What we’re looking for:

Our newest team member will be person-centred, compassionate, organised and excited to support our business in providing Level 3 Specialist Support Coordination.

While it is a full-time position, hours of work are flexible and you will have the ability to manage your own work schedule and work independently while having access to support and guidance in addition to weekly coaching sessions and having a 'team day' every Friday.

The ideal applicant is motivated, driven, has good attention to detail, excellent report writing skills and takes pride in their work. They are determined, energising and able to overcome challenges while maintaining a positive attitude and they genuinely and authentically care about the wellbeing of people with disabilities and their families.

The successful applicant will:

Provide level 3 Specialist Support Coordination to participants supported by Circle SC. Participants will have a variety of disabilities and will generally be living with complex support needs or be experiencing complex life situations.

Hold aligned values with Circle SC in relation to providing person-centred support to people with disability.

Build strong relationships and work closely with participants and their family.

Educate and develop understanding of the NDIS and use of NDIS plans.

Strengthen informal and community networks.

Identify and engage with high quality NDIS providers.

Support participants to resolve conflicts and work through challenges.

Take a ‘bird's eye’ perspective to holistic coordination.

Collaborate with the person’s support network and other providers to ensure everyone is ‘on the same page’ to most effectively support the participant.

Identify and reduce barriers to implementing a participant’s NDIS plan.

Focus on building the participant’s capacity.

Support the participant to navigate the NDIS plan reassessment process and get a positive outcome.

Write high quality reports and have the ability to communicate complex issues using ‘NDIS language’ in a clear concise way.

Keep accurate and detailed records (e.g progress notes) on our practice management system.

Proactive engagement in ongoing skill development and continuous improvement.

Be able to assertively set boundaries and implement self-care strategies.

Contribute positively to team culture, celebrate others success and take ownership over mistakes or setbacks.

Manage competing priorities and meet deadlines in a fast paced environment.

Supporting participants and families to achieve their goals by identifying and reducing any barriers to implementing their NDIS plan.

Work collaboratively with the participant, family, caregivers, the National Disability Insurance Agency (NDIA), mainstream government agencies, disability service providers, and the community to identify relevant supports and services to meet the participant’s NDIS plan goals.

Work with each participant to assess and choose their preferred provider, negotiate the services to be provided, develop pricing and service agreements, and create service bookings with agreed providers.

Liaise with participants and Plan Managers to establish best use of funding to maximise effective support and attribute the correct amount of funds to different support categories.

Employee skills, experience and qualifications

NDIS Specialist Support Coordinators are required to hold relevant university level qualifications and deep experience in the disability/healthcare/aged care sector.

Selection Criteria:

University level degree qualified in a relevant field (e.g. Social Work, Nursing, Psychology, other Allied Health, Health Sciences, Criminology, Addiction).

Good knowledge of, and experience supporting people living with disabilities.

In-depth understanding of the NDIS system.

High quality report writing skills.

Strong administrative, organisational, interpersonal and time management skills.

Desirable:

Lived experience with a disability including through family, friend or community networks.

Aboriginal and Torres Strait Islander People.

Previous experience as a Specialist Support Coordinator or complex case manager.

Accreditation with relevant professional body (AHPRA, APNA, AASW etc.)

Essentials:

Driver's licence and privately-owned vehicle.

Working With Children’s Check.

National Police Clearance.

NDIS Worker Screening Check.

Right to work in Australia.
