We are looking for a suitably qualified Specialist Support Coordinator/Support Coordinator to join our team in Broome. The successful applicants will be hand-picked based on their passion and commitment to creating change in our community.

The Role - NDIS Specialist Support Coordinator/Support Coordinator - Located in Broome
NDIS Support Coordinator
The primary role of the NDIS Support Coordinator is to provide non-clinical care and practical support to eligible individuals with a NDIS Plan to strengthen their ability to coordinate and implement supports, and participate more fully in the community.
Duties:
Broker supports and services in line with a participant’s wishes and their plan budget.
Monitor NDIS plan budgets and support effectiveness.
Build participants capacity and capability to understand their plan, navigate the NDIS, and make their own decisions.
Support participants to put in place a mix of supports to increase a participant’s capacity to maintain relationships, manage tasks, live more independently, and be included in their community.
Build the participant’s confidence and skills to direct their own lives,
Maintain integrity by declaring and avoiding any real or perceived conflicts of interest.
NDIS Specialist Support Coordinator
Specialist support coordination is a higher level of support. The focus is on reducing complexity in the participant’s support environment and helping the participant overcome immediate and/or significant barriers in plan implementation.
In addition to the duties outlined in the Support Coordinator role, a Specialist Support Coordinator’s duties include:
Addressing complex barriers that affect participants' access to support.
Establishing a process to overcome these barriers, including prioritising factors that need to be addressed first and what supports need to be in place immediately.
Ensuring a consistent delivery of service and access to relevant support in a crisis or unplanned event.
Selection Criteria
Demonstrated high-level skills and ability to engage and build rapport with NDIS participants, family members and service providers to identify solutions, empower, and build capacity.
Demonstrated understanding of, and commitment to, person-centered practice with excellent customer service outcomes.
Experience liaising with external service providers such as Guardians, GP's, allied health providers and other community-based mainstream services and organisations.
Proven experience in developing productive networks and working relationships.
Ability to prioritise work, manage time and meet deadlines independently; and
Strong written and verbal communication skills
Requirements
A tertiary qualification in community services, health services, or allied health is essential for the Specialist Support Coordinator role.
Must have a working knowledge of the NDIS, its principles and aims
Prior experience in providing professional and flexible assistance to individuals with disabilities.
Valid Driver's License and reliable vehicle
Current First Aid Certificate

NDIS Worker Screening Check
Willingness to travel to remote communities
