QHC is looking for an experienced Support Coordinator who can work with individuals, their families, friends, and supports to achieve set goals to improve the participants quality of life. The successful candidate will be responsible for carrying their own case load, managing participants with varied disabilities and complexities. You will be delivering services to NDIS participants with Quality health care’s approach to Support coordination in line with NDIS requirements. Service delivery is made up of a flexible mix of working from home, office based (in Hurstville) and client facing.

Your Role

• Assist participants in understand and utilising their NDIS plan funding.
• Work with participants to identify services that meet their needs and support their goals.
• Coordinate a range of supports using a strength-based approach across all areas of the participants life.
• Ensure the participants have choice and control in selecting providers and build their capacity to self-direct.
• Manage ongoing support, including resolving issues between participants and providers.
• Liase with government and non-government bodies, oversee budgets, obtain quotes and submit progress reports to the NDIA.

Essential Criteria

• Relevant tertiary qualifications or certificate in relevant field e.g. Social work, community services or allied health.
• Minimum 2 years experience in providing services to NDIS participants as a Support Coordinator
• Experience in reading, understanding, and implementing NDIS plans
• Experience in connecting clients to services under a client direct care model
• Excellent interpersonal and communication skills and a strong commitment to customer service
• Ability to manage a diverse participant portfolio
• Ability to maintain positive relationships with providers & community agencies
• Strong customer service skills with demonstrated ability to develop and maintain relationships with people with disability and/ or their families and support network and service providers
• Resilience and competence in high level interpersonal communication, negotiation and conflict resolution skills
• Sound time management skills and demonstrated ability to work with minimal supervision
• Demonstrated ability to make complex judgements, prioritise tasks and take initiative
• Strong written communication skills with the ability to write high quality reports
• Current driver's license and willingness to travel to clients where required
• Previous experience in a billable environment highly desirable
