About Us

South West Community Care is a not-for-profit Organisation and Registered Charity local to the South West, providing aged care and in-home services to the local community. It is managed by a Chief Executive Officer (CEO) who is supported by a Chief Governance Officer and Management Team overseeing operations and supporting a team of over 120 employees and volunteers. The CEO, in conjunction with a volunteer Board of Management, sets the strategic direction for the organisation.

South West Community Care is managed locally and embraces the ‘Wellness’ Approach when delivering care to eligible older Australians in the Community approximately 1000 client currently, providing services designed to maintain independence dignity and quality of life for our ageing community. These services are delivered by compassionate and caring staff working within the guidelines set out by the Aged Care Charter of Rights and Code of Conduct for Aged Care to deliver person centred care to optimise positive outcomes.

Position Purpose

Reporting to the Quality and Client Experience Manager, the purpose of your role is to provide administrative support to the Operational teams to ensure they are supported to work with diverse and changing priorities involved in provision of client centred care. This is a multidisciplinary role which assists in the implementation of care planning, service delivery and operational objectives, and provides support for the Quality and Client Experience Manager to ensure targets and objectives can be met.

Position Responsibilities

As an Operations Support Administrative Officer you will be responsible for and assisting with:

  • Our Clients: Preparation of client initial and ongoing assessments, respond to client incident management and completion of client commencement and exit documentation and as per SWCC policies and procedures.
  • Client Care Requests: Work with schedulers and contractors to coordinate service delivery timeframes.
  • Allied Health Requests: Provide support in the liaison with allied health professionals and healthcare facilities to assist in care planning and care services.
  • Accuracy and Confidentiality: Maintaining accurate records and ensuring confidentiality.
  • Care Team Support: Administrative and clerical support is provided through monitoring, ordering and collecting of consumables and supplies, and assisting in general office duties as directed.
  • SWCC Client Management System: Maintain accurate, up-to-date and professional case notes.
  • MAC Portal: Ensure compliance with referral deadlines and monitor incoming services.
  • Care Coordinator Support: Provide relief coverage.
  • First Point of Contact: When relieving on reception you will be the front-line of SWCC. You will present the first impression of our organisation through our reception and telephone contact points. You will interact with our client, members of the public, and our employees.

To thrive in this role you will have:

  • Certificate III in Business/Administration or equivalent.
  • Proven experience in business and/or customer service, including the ability to problem solve and provide exceptional customer service to a diverse range of clients.
  • Strong communication skills with the ability to manage a broad range of daily activities and deadlines.
  • Ability to work with a high degree of professionalism, discretion, confidentiality, and independence.
  • Experience using Microsoft 365 including Excel at a basic level.
  • Demonstrated ability to follow process and systems while being adaptable to the ever-changing landscape of our clients and their needs
  • Well-developed oral and written communication skills, and strong interpersonal and team skills.
  • Demonstrated experience and willingness to provide relief coverage of the reception.

Office Base Location

Based at the Australind Office but will be required to work from other SWCC sites.
