About the Role
Join our growing team in Darwin on a full time 12-month contract basis with high opportunity for renewals. Demonstrate organisational and leadership skills in the coordination of services to assist people with a disability in a community based environment. The position is focused upon ensuring that the individual needs of clients are met, whilst complying with the NDIS framework and maintaining an efficient and safe working environment for team members.
Key Responsibilities
Ensure the team complies with the Disability Services Act Standards, NDIS framework and other relevant legislation
Apply person-centered approach and a solution focused and strength approach
Coordination and management of the day-to-day operations of the community access and drop-in support disability programs
Coordination and development of clients' care plans in liaison with families and other stakeholders
Maintaining of participants records and providing updates on progress in weekly, monthly or quarterly reports and stakeholder meetings
Coordinate and oversee rostering and timesheet requirements according to client support needs
Provide supervision and support to team members
Provide daily care, supervision and transport to clients
Develop and support individual care plans, including medication, hygiene and meal requirements
Providing support to work towards individual goals
Encourage community engagement to support and increase life skill of clients
Development of professional relationships with the participants, their families and other stakeholders and engagement and regular stakeholder meetings
Provision of accurate and timely case notes and incident reports
Use of Brevity Care Software
Skills & Experience
Cert IV or higher in Disability, Mental Health, Social Work or other relevant qualification
Demonstrated experience of working in a leadership/management role in a human services organisation
Demonstrated experience in delivery of services to people with a disability in a community access and drop-in support environment
Strong coordination, problem solving and leadership skills
Experience supporting people with complex disabilities where challenging behaviours are common - TCI training desirable
Knowledge of trauma and its effects on the development and behaviours of people
The ability to contribute positively within a team
NDIS knowledge and awareness
Excellent communication skills, including computer skills, internet, email, Microsoft Word, Excel and PowerPoint
A positive mindset, proactive can do attitude, solution focused
Basic business understanding and the importance of professional attitude and meeting deadlines
Completion of NDIS mandatory training modules
A current First Aid Certificate
A current unrestricted NT Driver License
Availability to work on a rotating on-call roster
Successful candidates will be required to clear probity checks including National Criminal History Record Check, Working with Children Check, NDIS worker screening check.
