About the role

Join Pivot Support Services' dynamic and caring NDIS team as a Support Coordinator in our Albany, WA office.

As a Support Coordinator, you play a key role in enabling NDIS participants to live independently and thrive in their communities. This position offers opportunities for professional growth and development within a supportive, values-driven organisation.

The role:

Develop and coordinate individualised support plans to help clients achieve their goals

Liaise with service providers, community groups and stakeholders to arrange and monitor services

Advocate for client needs and ensure they receive high-quality, person-centered care

Review and update supports to reflect changing needs and circumstances

Maintain detailed client records and documentation in accordance with organisational and legislative requirements

Provide assistance to link clients, their families and carers to supports available in the community

Identify opportunities to improve service delivery

What we're looking for

Relevant tertiary qualification in community services, disability support or a related field

Preferred - experience in a similar support coordination or case management role

Strong understanding of the disability support sector and the NDIS

Excellent communication, interpersonal and advocacy skills

Ability to work collaboratively with a diverse range of stakeholders

Strong organisational and time management skills

Empathetic, person-centered approach as well as a commitment to delivering high-quality client outcomes

Current driver's license

NDIS Worker Screening Check or willingness to undergo screening
