Sanctuary Family Connections | Hunter New England Region
Full-Time | NDIS Disability & Youth Services

About Us
At Sanctuary Family Connections, we’re driven by purpose. We provide trauma-informed, strengths-based support to children, young people, and adults living with disability. Whether it’s supporting a young person transitioning from Out of Home Care or empowering adults to live more independently, our focus is always on connection, capacity-building, and community.

We’re proudly raising the standard in NDIS service delivery. Our supports are built on empathy, consistency, and integrity. We believe in working alongside participants, families, and support networks to create environments where individuals feel seen, valued, and safe.

The Role
We are seeking an experienced and compassionate Team Leader to join our leadership team in the Hunter New England region. You will oversee Supported Independent Living (SIL) placements and Community Access supports, leading your team to deliver high-quality, person-centered care that aligns with participant goals and promotes independence and wellbeing.

Reporting directly to the Program Manager, you’ll play a pivotal role in guiding frontline staff, maintaining service excellence, and ensuring that daily operations, compliance, and therapeutic practices are upheld with care and consistency.

What You’ll Be Doing

Providing day-to-day leadership, coaching, and mentoring to your team of Disability Support Workers

Monitoring participant wellbeing, ensuring daily routines, medication, appointments, and personal goals are consistently supported and documented

Supporting your team to embed trauma-informed practices in every interaction

Coordinating household operations, including food planning, hygiene, safety, and documentation oversight

Advocating for participants’ best interests during family time and across the care network

Collaborating with health professionals, guardians, and case managers to meet participants’ health and behavioural needs

Facilitating team meetings, conducting supervision, and contributing to reflective practice

Conducting risk assessments, supporting WHS compliance, and managing incident reporting

Tracking budgets and ensuring all financial tasks (receipts, reimbursements, spending approvals) are completed in collaboration with the Program Manager

Leading with authenticity and promoting our organisational values at every touchpoint

About You

Experience in a senior or leadership role within disability, youth, or NDIS sectors

Solid understanding of Supported Independent Living, Community Access, and participant goal planning under the NDIS

Warm, reliable, and strengths-focused leadership style

Resilient, organised, and emotionally intelligent – able to manage competing priorities with calm and care

Proficient in documentation, report writing, and staff communication systems (Teams, shift reports, compliance tools)

Enthusiastic about growing team capability and improving service delivery

Essential Criteria

Cert IV or higher in Disability, Community Services, or related field

Demonstrated experience supporting NDIS participants, ideally in SIL settings

Current Working With Children Check (NSW), National Police Check, and First Aid/CPR

Valid Driver’s Licence and access to a reliable vehicle

Why Join Sanctuary Family Connections?

Be part of a values-led organisation that champions care, collaboration, and connection

Work in a team where your voice is valued, and your growth is nurtured
