Fba Care is looking for a committed and passionate Community Nurse to join our growing team in Tottenham Victoria.

The Community Nurse delivers clinical care within the community, including personal care, medication administration, and health assessments.

Responsibilities:

Provide clinical nursing services within the Community.

Provide physical assistance as required. This may include assistance with all areas of personal care including feeding, toileting, bathing and grooming, transfers/hoisting between toilet,

Wheelchair, motor vehicle, etc. and administering medication or other medical/special procedures where required

Assist as directed in the development of patient care plans, health assessments and the implementation of other health initiatives

Assist in the delivery of person-centered service according to the objectives of each program

Induct and train new staff in regard to healthcare planning

Assess, review, develop and educate staff on individual health care plans and procedures

Support staff in medication administration, and support quality improvement processes

Identify early preventative actions for health concerns that may impact on other associated issues such as behavior

Establish effective communication with participants using whatever means of communication is appropriate for each individual and liaise with families and or advocates when required

Delivery of excellent nursing care in accordance with the philosophy of person centered, coordinated care

Practice complies with national code for health care workers

Ensure all service expenditure has approval in line with FBA Care’s policy and guidelines

Continually monitor billable hours associated with Support Coordination service delivery

Participate in scheduled supervision with related supervisor

Undertake annual perform professional appraisals with the related supervisor and HR and develop and implement own professional development plans

Attend and contribute to care team meetings, supervision meetings, and other relevant meetings as required and use information/feedback to continuously improve work performance

As per FBA Care policy and procedures ensure all mandatory reporting requirements are adhered to

Ensure all service delivery meets all legislative requirements

All other duties as directed

Essential Criteria:

The ability to provide comprehensive and coordinated nursing services in the general practice setting

Strong knowledge and understanding of the NDIS

Excellent verbal and written communication skills

Exceptional time management skills

Ability to adapt and respond to participants' needs

Demonstrated competence in delivering services that ensures participants are safe

Ability to demonstrate the highest level of customer service and response

Demonstrated commitment to quality improvement and evidenced based practice

Proven ability to handle various priorities and work in a proactive manner, organizing and managing own workload

Strong problem-solving skills

Demonstrate reliability, punctuality, and trustworthiness

Strong commitment to a person-centered approach

Ability to work independently with disabled individuals to achieve social and self-care goals

Positive attitude with nurturing and kind nature

Desired Criteria:

Bilingual

Proven experience in the disability sector

Tertiary qualification in Nursing and/or previous experience in similar role

Current AHPRA registration
