About the role

We are seeking an experienced Case Manager to join our team at BRADAAG' in Tennant Creek, NT. As a Case Manager, you will play a crucial role in supporting individuals and families within our community. This is a full-time position that offers the opportunity to make a meaningful impact.

What you'll be doing

Providing comprehensive case management services to clients, including assessment, care planning, coordination, and monitoring

Advocating for clients and liaising with relevant stakeholders to ensure access to appropriate services and support

Maintaining accurate and up-to-date case notes and records in line with organisational policies and procedures

Collaborating with a multidisciplinary team to deliver holistic and integrated support

Participating in regular supervision and professional development activities

Adhering to all relevant legislation, regulations, and industry standards

What we're looking for

Relevant tertiary qualifications in Community Services, Alcohol and Drugs, Mental Health, Counselling, Social Work, or a relevant course with a focus on Drugs and Alcohol, or willingness to obtain.

Demonstrated experience in case management, preferably in the AOD or family services sectors

Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and stakeholders

A proven track record of working effectively within a multidisciplinary team environment

Strong organisational and time management skills, with the ability to prioritise and manage a diverse workload

A deep understanding of the challenges faced by vulnerable individuals and families, and a commitment to promoting their wellbeing
