Make a Difference with Burdekin
We are seeking a passionate and empathetic Case Manager to join our Inner West team at Burdekin on a 12 month contract. As a Case Manager, you will be working alongside a team of Case Managers, Supportive Specialists, Carers and Youth Workers.
You will provide person-centred case management to young people who come to our service, to ensure that the young person’s day-to-day needs are met, encourage and support active participation in education & program activities, and facilitate development of the young person’s personal & living skills.
As a Case Manager you will hold a small case load or 4-7 clients across a range of programs such as Supported Independent Living, Staffed and carer-based models.
Essential Criteria
- Minimum Diploma/Degree qualification in Social Work, Social Welfare, Psychology or related fields
- Minimum of one year case management experience
- Knowledge, skills and experience working with young people and their families in a respectful and caring manner
- Demonstrated ability to write reports to a professional standard
- Ability to work independently and as part of a team
- Available to participate in a rotating after hours on-call roster
- National Police Check & Working with Children Check or willingness to obtain
- Must have a valid driver's licence, your own vehicle and comprehensive insurance
- Permanent work rights in Australia
