Salary Level: Level 3-4 (increment level negotiated based on skills and experience)

Award: Social, Community, Home Care and Disability Services Award

Location: Mount Isa, Queensland

Reports to: Reporting to the Senior Case Worker

Status: Permanent Full-Time

YHARS – Youth Housing and Reintegration Services Purpose:

YHARS is an initiative under the Homelessness National Partnership Agreement between the Commonwealth Government and the Queensland Government. The initiative provides a coordinated and integrated response to the complex needs of young people who are homeless or at risk-of homelessness. YHARS targets young people aged 12-21 years who are transitioning from a child safety or youth justice order, or you have been “sleeping rough” or living in unstable or temporary housing arrangements.

Child Safety Support in Supported Independent Living (SIL) Units – Purpose:

The purpose of child safety support in SIL units is to ensure that young residents are provided with a safe, secure, and nurturing environment while developing independent living skills. This support focuses on protecting children and young people from harm, promoting their well-being, and empowering them to live as independently as possible, all while ensuring that their safety and emotional needs are met.

Key Responsibilities and Accountabilities:

1. Policy and Procedure Compliance: Work under general direction to ensure the application of organisational policies, procedures, work practices, and specific program requirements. Maintain adherence to the guidelines for both YHARS and SIL programs to ensure the effective delivery of services in both areas.

2. Work Planning and Management: Manage and plan your own workload and that of volunteers across both YHARS and SIL programs. Ensure effective delivery of case management support that promotes positive engagement, well-being, and independence for young people.

3. Case Management and Support: Address the underlying causes of homelessness and related challenges by providing comprehensive case management and support to young people and their families. Focus on issues such as domestic violence, intergenerational disadvantage, mental health, criminality, and economic or social exclusion. Provide tailored interventions across both YHARS and SIL programs to meet individual needs.

4. Service Delivery and Program Implementation: Develop, implement, and monitor case plans that are responsive to the needs of young people in both YHARS and SIL settings. Ensure the effective delivery of support services, including assistance with housing, life skills development, emotional support, and access to relevant community resources.

5. Collaborative Partnerships: Establish and maintain strong working relationships with local community organisations, government agencies, and key stakeholders such as the Department of Child Safety and the Department of Youth Justice. Collaboration is essential in delivering integrated support for young people across both YHARS and SIL programs.

6. Youth Development and Empowerment: Support youth development by providing opportunities for young people to build essential life skills, increase independence, and achieve their personal goals. Facilitate access to community resources and services that promote youth empowerment and well-being in both YHARS and SIL programs.

7. Responsive Case Planning and Monitoring: Develop and adjust case plans that are responsive to the changing needs of young people. Ensure that case management is individualised, proactive, and regularly reviewed to track progress and adjust interventions as necessary to support the client’s journey towards independence.


Are you the right person for the job? (Skills and Competencies)

Qualification and Experience:

Bachelor’s degree in Social Work, Community Services, Psychology, or a related field, or

Diploma or Certificate-level qualification in a relevant area, accompanied by practical experience in working with youth and case management, or

Extensive relevant vocational or lived experience in working with youth and providing case management support.

Knowledge:

Demonstrated understanding of youth and social justice issues, and the service delivery required to support and develop young people and their families.

Demonstrated understanding of youth issues, including homelessness, mental health, and intergenerational trauma, with a focus on supporting youth through case management interventions.

Proven knowledge in case planning and delivering support services to youth in both YHARS and SIL programs, ensuring a holistic and client-centred approach.

Skills:

Strong communication skills, both verbal and written, with experience in collaborating effectively with government departments, community organisations, and service providers to plan and implement cultural programs across both YHARS and SIL.

Ability to design and deliver culturally relevant activities and programs that meet the diverse needs of young people, fostering a sense of belonging, empowerment, and inclusion across both programs.

Aptitude:

Demonstrated commitment to supporting youth well-being through case management and providing comprehensive, responsive services across both YHARS and SIL programs.

Proven ability to lead or support case planning, coordinating with a range of professionals to ensure optimal outcomes for young people.

Demonstrated ability to work in a professional and ethical manner, particularly regarding accountability, client confidentiality, and duty of care.

Proven ability to work cooperatively and proactively within multidisciplinary teams across both YHARS and SIL programs.

Demonstrated self-awareness and a strong commitment to ongoing professional development.
