Service Coordinator | Disability & Community Care

Employment Status: Full- time

Location: Toowoomba, Lockyer & Surrounding Areas

Main responsibilities include:

As Big Smile Service Coordinator, you will oversee the delivery of best practice, quality care within our service coverage

Develop and follow strategies to bring new business/clients

Proven ability to develop and maintain effective working relationships with stakeholders at all levels. Liaising with key Hospitals, Support Coordinators, and other relevant stakeholders.

Have a strong network of referral contacts

Undertake activities to grow inquiries which may include expos, networking events, presentations to referrer groups and meetings with potential new referrers

Ensure participant care is assessed, planned, implemented, and evaluated in accordance with Big Smile Services, as well as funding models, taking into consideration each client’s physical, social, emotional, spiritual, and clinical needs and encouraging participant/ participant representative participation.

Complete regular review of services ensuring effective schedule of client services and maximising package spends.

Act as a key contact for participant through their Big Smile Journey – and be a day-to-day entry point for services and general enquiries, plan enhancements and changes to funding.

Onboarding and offboarding participants, providing a clear explanation of our services and ensuring they feel confident in their decision to choose Big Smile Services

Conduct risk assessments and implement strategies to minimize potential risks, ensuring the safety and well-being of participants. Collaborate with participants' support providers, networks, and family members to deliver comprehensive care.

Assist with interview and orientation of new staff and ongoing professional development. Fostering and driving a high-performance culture and providing great quality of life aligned to the Big Smile Services values and mission.

Representing Big Smile Services in a positive and responsive manner

Administration & Compliance:

Maintain accurate and up-to-date records of participant interactions, service agreements, and plan progress. As well as Employee files.

Ensure compliance with NDIS guidelines, policies, and procedures.

Prepare and submit reports as required, including progress reports and incident reports.

What makes a difference for us?

Certificate IV or higher qualifications in Disability , Community Services or Human services,

1-2 years’ demonstrated experience in service coordinator role or similar will be highly regarded.

Demonstrated experience in rostering and coordinating the delivery of disability and/or community services.

Knowledge of NDIS processes and Community Services Funding Models.

Working knowledge of best practice rostering, workforce optimization and system utilization.

Experience in managing workforce issues including supervision, conflict resolution and mentoring.

Experience in incident management and reporting.

Extensive stakeholder management capabilities and delivering outcomes

Ability to deliver consistent professional and innovative person – centred care

Excellent written and verbal communication skills

Other Requirements

Successful applicants will also need to hold a ‘C’ class Open driver licence, State Working with Children Check and hold, Workers Screening Check and Current National Police Clearance or ability to obtain.
