- NDIS Support Coordinator
Responsibilities:
- Understanding and Implementing NDIS Plan
- Connect participants with appropriate services and supports
- Monitoring progress and Reviewing Supports
- Provide regular reports and maintain accurate records
- Work collaboratively with families, service providers, and stakeholders
- Navigating the NDIS System
- Resolving Issues and Managing Crises
- Building Capacity and Independence
- Coordinating Supports
Skills & Qualifications:
- Relevant qualifications (social work, psychology, community/disability services or similar)
- Experience in disability, social work, or community services.
- Strong knowledge of NDIS plans and processes.
- Excellent communication and problem-solving skills.
- Ability to work independently and as part of a team.
- Strong organizational and time-management abilities.
- Ability in upholding ethical standards and maintaining professional boundaries.
- Ability to work towards, and meet billable and KPI targets
- Comprehensive understanding of NDIS framework and policies will be highly regarded.
2. NDIS Specialist Support Coordinator
Job Summary:
We are seeking an experienced NDIS Specialist Support Coordinator to work with participants who have complex needs, including psychosocial disabilities, high risk situations or multiple service requirements. The role involves providing high-level support coordination, problem solving and crisis management to ensure participants receive the best possible outcomes from their NDIS plans.
Responsibilities:
1. Implementing and Managing NDIS Plans
2. Complex Case Management and Crisis Intervention
3. Coordinating and Monitoring Specialist Supports
4. Liaise with healthcare professionals, legal services, and government agencies.
5. Regularly Reviewing Plans and Recommending adjustments.
6. Collaboration with Families, Service Providers and Stakeholders.
7. Compliance, Reporting and Documentation
8. Advocacy and Problem-Solving
Skills & Qualifications:
- Relevant qualifications (social work, psychology, community/disability services or similar)
- Extensive experience in disability services, mental health, or allied health.
- In-depth knowledge of NDIS frameworks and complex case management.
- Strong knowledge of NDIS plans and processes.

- Support Coordinator Manager (COS Manager)
Job Summary:
We are seeking an experienced Support Coordinator Manager (COS Manager) to lead and oversee a team of NDIS Support Coordinators. The role involves managing service delivery, ensuring compliance, supporting complex cases and driving quality outcomes for NDIS participants. The COS Manager will also focus on continuous improvement, stakeholder engagement and operational efficiency.
Responsibilities:
- Leadership and Team Management.
- Oversee, manage and support a team of Support Coordinators and Specialist Support Coordinators.
- Provide supervision, training and professional development opportunities for staff.
- Conduct performance evaluations set goals and manage workloads.
- Foster a positive and supportive team environment.
- Service Delivery and Quality Assurance
- Ensure high-quality person-cantered support coordination services
- Implement best practices to enhance participant outcomes and service effectiveness.
- Conduct case reviews and audits to assess service quality and compliance.
- Address service gaps and continuously improve service delivery.
- Compliance and Reporting.
- Ensure all support coordination activities comply with NDIS guidelines, policies and regulations
- Over see accurate records-keeping and monitor documentation standards.
- Prepare reports for internal stakeholders and regulatory bodies as required
- Complex Case Management and Escalations
- Assist with high-risk and complex participant cases that require advanced coordination
- Provide guidance and risk management strategies for escalated participants concerns
- Advocate for participants in disputes, appeals or service provider negotiations.
- Manage KPIs of Support coordinators
- Set, track and manage KPI’S for support Coordinators.
- Ensure Support Coordinators meet service delivery targets and maintain high-quality outcomes
- Provide performance feedback and implement improvement strategies as needed.
- Compliance with NDIS standards and VHCC policies.
- Ensure all support coordination activities comply with NDIS guidelines and VHCC policies.
- Oversee accurate records-keeping and monitor documentation standards.
- Maintain up-to-date knowledge of regulatory changes and ensure staff adherence
- Stakeholder Engagement and Relationship Management
- Build and maintain strong relationships with NDIS participants, families and service providers
- Liaise with healthcare professionals, legal services and government agencies to enhance participant outcomes
- Represent the organisation in industry meetings, forums and networking events
- Continuous Improvement and Business Development
- Identify areas for service improvement and implement new strategies
- Develop and refine internal processes to enhance efficiency and effectiveness
- Stay up to date with NDIS policy changes and industry trends to ensure compliance and innovation
- Financial and Resource Management
- Oversee service allocation and ensure efficient utilisation of NDIS funding
- Ensure services align with financial sustainability and budget requirements
- Assist with pricing, billing and financial reporting related to support coordination services
Skills & Qualifications:
