Qualifications:
- Minimum Certificate-III in Disability/ Mental Health/ Youth Work/ Community Service. Preferred diploma/bachelor’s degrees.
- Have and maintain an Australian driver's Licence and reliable transport with comprehensive insurance.
- Obtain and maintain a First Aid & CPR certificate.
- Obtain and maintain a NDIS Workers Check Clearance.
- Obtain and maintain a Working with Children Check.
- Proof of right to work in Australia.
Selection Criteria
- Previous experience in NDIS space.
- Demonstrated proficient written, verbal, and interpersonal communication skills.
- Demonstrated ability to work with minimal supervision, and collaboratively as a member of a multidisciplinary team.
- Ability to effectively prioritise and organise own work / caseload.
Tasks may include but not be limited to the following.
Client/NDIS Participant Onboarding:
- Respond to client inquiries (phone calls, emails, walk-ins etc) regarding services.
- Conduct initial consultations to assess client needs, goals, and eligibility under the NDIS.
- Schedule initial assessments, intake meetings, and follow-up sessions as required.
- Create initial service agreements and coordinate with clients to sign off in terms of service.
- Prepare a Roster of Care (ROC) and share it with all relevant stakeholders.
- Conduct home and participant risk assessments before service delivery.
Documentation and Compliance:
- Collect, verify, and file necessary client documents such as NDIS goals/plans, identification, allied health reports etc
- Ensure all client records are accurately maintained and compliant with NDIS and ADCS policies and procedures.
Liaison with Stakeholders:
- Coordinate with internal and external teams such as service coordinators, allied health professionals, support workers, guardians, Local Area Coordinators (LAC), NDIS planners etc to facilitate seamless service provision.
New participant sourcing and business development:
- Organise different events and meet with different stakeholders to source new participants and create brand awareness.
- Working towards meeting the set KPI.
Participant Support:
- Offer ongoing support to clients during the intake process, ensuring a smooth transition into receiving services.
- Respond to queries and concerns from participants, families, and carers regarding their service needs.
- Follow up with participants post-intake to ensure satisfaction and ongoing engagement.
Reporting and Continuous Improvement:
- Prepare intake and progress reports as required by management.
- Participate in quality assurance activities to improve the intake process.
- Provide feedback to management on any issues affecting service delivery or client satisfaction.
